Visa for your convenience. After the authorization is complete, our website will confirm the order and you will receive an emailed receipt momentarily.
Vital Statistics Information
To obtain the permit to perform the cremation, we need to file the death certificate in the county where death occurred. Our secure website allows
you to input this information directly to us so that we can complete and file the death certificate on your behalf.
Alternatively you can download this form or request that it be mailed to you so you can complete it and return it at a later time.
Authorization for Cremation
The authorization for cremation form legally authorizes us to perform the cremation. This form must be signed by the legal next-of-kin(s),
also known as the authorizing agent(s). To determine who the authorizing agent(s) is, click here or contact our office for guidance.
The Authorization for Cremation and Disposition form must be signed in front of a witness. If there is no witness signature, the form is invalid and
we cannot perform the cremation.
This form can be downloaded on the last screen and signed by the next of kin(s) with highest legal standing, witnessed and returned to our office.
