We offer two pre-need direct cremation packages: Worldwide and Local. These two packages provide exactly the same services and products; the only difference is one covers when death occurs anywhere in the world versus the other which covers when death occurs within 75 miles of your residence.

To obtain the permit to perform the cremation, we need to file the death certificate in the county where death occurred. Our secure website allows you to input this information directly to us so that we can complete and file the death certificate when the time comes.
Alternatively you can download this form or request that it be mailed to you so you can complete it and return it at a later time.
There is no better person to authorize his or her cremation than you! This form memorializes your intention for your body to be cremated at the time of your death. Keep a copy once completed and witnessed for your own records.

To memorialize your preplanned cremation wishes, there are documents that we will prepare based on your choices online. These documents should arrive within one to three business days for your review and signature. Please return these to our office in the postage provided reply envelope.
Note: if the documents are not returned to our office within 30 days, we will be forced to cancel your cremation plans and refund all funds back to the card you used. This is to comply with state law.
